Primelight Cradle Care Servises is preparing to offer person-centered supports for adults with intellectual and developmental disabilities in Maryland. Planned services are designed to promote indepencence, individual choice, safety, community participation, and quality of life. Services will be provided based on individual needs , eligibility, applicable authorizations, services planes, and program requirements.
Services & Supports
Community Development Services
Planned Community Development Services will focus on helping individuals develop skills, build relationships, participate in meaningful activities, and increase community involvement based on their goals, interests and choices.
Personal Supports
Planned Personal Support will focus on individualized assistance with daily living activities, skill development, community participation, and personal goals while promoting independence and respecting each person's preferences and choices.
Supported Living
Planned Supported Living Services will provide person-centered assistance designed to help individuals develop and maintain the skills needed to live as independently as possible in their homes and communities.
Respite Care
Planned Respite Care will provide temporary, person-centered support intended to give families and caregivers periods of relief while promoting each individual's safety, dignity, preferences, choices, and needs. Services will be provided in accordance with applicable requirements and authorizations.
Nursing Support
Planned nursing-related support will be based on each individual's assessed needs and authorized service plan and will be provided or coordinated by appropriately qualified professionals in accordance with applicable programm requirements.
Transportation Services
Planned Transportation Services will Support access to authorized services, community activities, appointments, and other appropriate destinations based on each individual's needs, goals, service plan, applicable authorizations and program requirements.
Therapy Coordination and Access
Primelight Cradle Care Servises paln to support access to physical therapy, occupational therapy, and speech-language pathology when clinically appropriate and authorized. Therapy services will be provided by appropriately licensed professionals and coordinated according to each individaul's assessed needs, applicable authorizations, and plansa of care.

Person-Centered Support
Our approach is designed around each persos need, preference, abilities, goals, and choices. When services are authorized, supports will be guided by the individual's approaved service plan and applicable requirements, with an emphasis on dignity, independence, safety, uindividual choice, and meaningful community participation.
